WAIVER, RELEASE OF LIABILITY, AND ASSUMPTION OF RISK

PLEASE READ CAREFULLY – THIS DOCUMENT AFFECTS YOUR LEGAL RIGHTS

I acknowledge that I have read, understand, and have explained to me that the martial arts known as Jiu-Jitsu, Gracie Jiu-Jitsu, Brazilian Jiu-Jitsu , and Valente Brothers JuJutsu are taught with all reasonable precautions taken to avoid injuries. However, I understand that participation in martial arts training involves physical contact and exertion , and therefore carries a risk of accidents and injury .

I understand that these risks may include serious injury, permanent disability, or even death as a result of practicing the martial arts mentioned above.

I further understand that participation may also involve risk of damage to personal property or financial loss , including but not limited to medical expenses, loss of wages, or loss of the ability to earn wages .

With full knowledge of these risks to my person and property, both present and future , and after having these risks fully explained to me and/or to my legal guardian, I voluntarily agree to participate in training.

Release of Liability

I hereby release, waive, and forever discharge from any liability the following entities and individuals:

• Jujutsu Self-Defense Systems

• Valente Brothers Jiu-Jitsu

• Pembroke Pines Jiu-Jitsu Inc.

as well as their agents, stockholders, directors, officers, employees, instructors, students, and representatives .

This release applies to any and all claims, demands, rights, or causes of action of any nature whatsoever , whether known or unknown, foreseen or unforeseen, that may arise from my participation in training or activities associated with these organizations.

Assumption of Risk

Understanding the risks involved, I voluntarily assume all risks associated with participation in martial arts training.

I agree to release and hold harmless the persons and entities listed above from any claims for damages or injuries that may occur as a result of my participation.

I further understand and agree that this Waiver, Release, and Assumption of Risk is binding on my heirs, family members, and legal representatives .

Acknowledgment

My signature below confirms that:

• I have read and understood this document.

• I fully understand the dangers inherent in martial arts training.

• I voluntarily agree not to hold Valente Brothers Jiu-Jitsu, Pembroke Pines Jiu-Jitsu Inc., or their agents and affiliates responsible for any injury, whether arising from training or related activities.
  

Media Release

Without expectation of compensation or other remuneration, now or in the future, I hereby grant permission to Valente Brothers Self-Defense, its affiliates, and agents to use my image, likeness, photographs, video recordings, and/or interview statements in publications, advertising, promotional materials, or other media activities, including print, video, and internet media .
